- Must have professional license and/or certification as required for the position with primary source verification within 30 days of start date and updated upon expiration.
- Must have documentation of TB skin test screening within 3 months of start date and updated if break >6 months, or negative QuantiFERON TB Test or T-spot within 3 months and updated if break >6 months.
- Must have physician health statement signed by MD/DO/NP/PA within 365 days of start date and updated if break >6 months.
- Must have MGB Criminal Background Check within 45 days prior to start date and updated if break >6 months.
- Must have MGB Massachusetts CORI background check within 30 days prior to start date and updated if break >6 months.
- Must provide MGB Resume including past 7 years of work experience with city and state.
- Must provide 2 references within 4 years of submission, minimum 1 supervisor or above in skillset.
- Must complete MGB Skills Checklist within 1 year of submission.
- Must complete MGB Confirmation of Assignment/POI Form for all initial assignments and extensions upon acceptance.
- Must have MGB Education Verification through background check company.
- Must have MGB SSN Trace with addresses/aliases for past 7 years.
- Must have MGB National Sex Offender Check within 30 days of start and updated annually.
- Must have MGB Exclusion Checks within 30 days of start and annually.
- If moving into shared MVH housing, must sign MGB MVH Housing Agreement.

- Positive PPD requires negative chest x-ray documentation.
