Case Management and Home Care RN registered nurse needed for PACE program located in Chula Vista and San Ysidro. The hiring organization is a good, supportive place to work with a positive mission contributing to the community. The focus of this role is case management and annual/semi-annual assessments of participants in a managed care program.
This position allows for partial work from home, in addition to in-home visits with program participants and team meetings at a central clinic location.

Requirements
Active RN License
Completed COVID vaccinations
