We are actively seeking a Social Worker Medical Discharge Planner to join our amazing team in our Transitional Care Unit and offering a $7,500 Sign on Bonus!

This is a Full Time Monday through Friday position. If you are interested the primary functions and requirements are listed below. This is a fast-paced role with a dynamic group of professionals and allows you to meet some amazing people!

Primary Duties and Responsibilities

• Completion of all required assessments, insurance reviews, and medical chart documentation

• Frequent interdisciplinary communication and meeting coordination

• Caseload management from admission to discharge

• Maintain effective, timely communication with patients and their families

• Understand and meet all government requirements for documentation

Skills and Qualifications

(Required)

• BA in Social Work or area of social sciences

• Excellent verbal and written communication skills

• Proven organizational skills

• Ability to work well in a fast-paced environment with competing priorities

• Exceptional time management skills

• Successfully able to negotiate working independently and in a team setting

(Preferred)

• Previous work with geriatric and diverse populations

• Knowledge of Medicare and Medicaid systems

• Insurance utilization reviews

• Familiarity with Point Click Care

• Knowledge of Minimal Data Set (MDS)

• Understanding of community services relevant to population
