An RN Care Manager, in collaboration with members of the healthcare team, leads the development and implementation of the interdisciplinary plan of care for patients, determining the appropriate level of care, supervising the provision of the discharge plan of care, and ensuring effective quality and cost-efficient outcomes by performing concurrent case review. This position functions as the key linkage between the physician, staff, and hospital leadership in the day to day management of appropriate and efficient care and functions as an advisor to the physician with accountability to escalate cases to the Clinical Manager (and higher as necessary) to ensure the provision of appropriate and effective patient care.

Must be a graduate of an accredited Nursing program; BSN preferred. Must have at least five (5) years

acute-care experience in Med/Surg and/or Critical Care. Utilization Review, Care/Case Management

experience in an acute care hospital setting preferred. Must have basic computer literacy. Working

knowledge of MS Word, Lotus Notes, and Clinical database software (i.e., InterQual) preferred. Must be a Registered Nurse. Must have a current DE License for RN. Certified in Utilization Management and/or Case Management preferred.
