Role Description
This is a full-time, on-site role located in the Atlanta Metropolitan Area for a Community Health Worker. The Community Health Worker will serve as a vital link between patients and healthcare providers to enhance care access, address barriers, and support individuals in navigating health and social services. The role involves building trusted relationships with community members, conducting outreach activities, educating patients on available resources, and gathering information to improve service delivery.
