**Position Overview:**

As a Case Worker for our Homeless Outreach Program, you will play a pivotal role in helping homeless individuals access the support and resources they need to transition out of homelessness. Your role will involve building relationships, assessing needs, and providing tailored assistance to create a pathway towards stability.

**Responsibilities:**

– Conduct outreach to engage with homeless individuals and build rapport

– Conduct comprehensive assessments to identify needs and barriers to stable housing

– Develop personalized case plans in collaboration with clients, setting achievable goals

– Connect clients with housing resources, employment, and social services

– Maintain accurate and up-to-date case notes and documentation

– Collaborate with a multidisciplinary team to ensure coordinated care

– Advocate for clients’ rights and needs within the community and social service systems

**Qualifications:**

-Associate’s degree in Human Services, psychology, or related field

– Previous experience working with homeless populations or in a related social services role

– Strong interpersonal and communication skills

– Empathy, patience, and cultural sensitivity

– Ability to work independently and as part of a team

– Knowledge of local resources and social service systems

– Proficient computer skills for documentation purposes

– Valid NY State driver’s license and reliable transportation
